Janine is one of those people who is very, very careful with her friends' feelings and very reckless with herself. She's just a sweetheart. And yes, unlike John, she is very forward with what she thinks, which I think Sherlock finds startling but also a bit refreshing at the same time.
And Janine really is just thinking of Sherlock, but it's true that her behavior has the happy side effect of making John *think.*
